Please pre-register for rehabilitative services by calling our office at 352.253.3892.  This will ensure benefit coverage prior to initiation of treatment.  Upon arrival, you will be asked for a valid Driver’s License (or other form of a photo ID), your current insurance card, and your physician's order form (provided by your referring physician – if you do not have a copy of the order, please make sure a copy has been faxed to the Rehabilitation Institute prior to your arrival). 

As a courtesy, if you must cancel or reschedule, please call 24 hours before your scheduled appointment time.
