Please pre-register for rehabilitative services by calling our office at 352.253.3892 or by following the link below to an online registration process.  This will ensure benefit coverage prior to initiation of treatment.  As a courtesy, if you must cancel or reschedule, please call 24 hours before your scheduled appointment time. Upon arrival, you will be asked for a valid Driver’s License (or other form of a photo ID), your current insurance card, and your physician's order form (provided by your referring physician – if you do not have a copy of the order, please make sure a copy has been faxed to the Rehabilitation Institute prior to your arrival).
